Independent Facilitator - Self-Determination Program
Job Summary/Objective:
We are seeking a dedicated and community oriented Independent Facilitator to support clients within the Alta Regional Center to participate in the Self-Determination Program. The role involves empowering individuals with developmental disabilities to exercise greater control and choice over their services and supports. As an Independent Facilitator, you will work closely with participants, their families, and service providers to facilitate person-centered planning and ensure the successful implementation of individualized plans.
Responsibilities:
• Conduct initial assessments to determine participants' needs, preferences, strengths, and goals.
• Collaborate with participants, families, friends, and support teams to develop comprehensive person-centered plans.
• Provide ongoing support and guidance to participants in enrolling in, and navigating the Self-Determination Program.
• Assist participants in accessing community resources, services, and supports that align with their goals.
• Advocate for participants' rights, preferences, and choices in all aspects of their lives.
• Monitor the implementation of individualized plans and make adjustments as needed.
• Maintain accurate documentation of participant progress, interactions, and outcomes.
• Facilitate communication and collaboration among all stakeholders involved in the participant's care.
• Stay informed about relevant laws, regulations, and best practices related to self-determination and disability services.
• Participate in training and professional development activities to enhance knowledge and skills.
• Act as a mandated reporter by adhering to legal and ethical guidelines; promptly report any suspected cases of abuse or neglect to appropriate authorities.
Requirements:
• Excellent communication, interpersonal, and advocacy skills.
• Ability to work independently and as part of a team.
• Strong proficiency in computer skills and documentation.
• Valid driver's license and reliable transportation.
• Flexibility, and a willingness to learn and adapt
• Willingness to obtain certification in person-centered planning.
• Independent Facilitator certification
Preferred Qualifications:
• Strong understanding of person-centered planning principles and practices.
• Previous experience working with individuals with developmental disabilities and/or in the field of disability services.
• Experience (through a family member, friend, or previous employer) working with Alta California Regional Center
• Bilingual proficiency in English and another language commonly spoken in the region.
